We value your pet’s health and well-being. Before we proceed with any surgical procedure, we require your authorization and some essential medical information. Please complete the Surgery Release Form below to ensure we have your consent and can deliver the safest care possible for your pet.

Please Read The Following Instructions Carefully

Start all medications the following morning.

Please return in:

Check incision daily for redness, swelling and/or drainage.

No bath for 10-14 days.

Limit exercise for 10-14 days. No running or jumping. Leash walks only.

Protect from undue exposure, such as getting wet, chilled, or overheated.

Cats should be kept indoors at all times.

Declawed Cats: Use Shredded Newspaper or Yesterday's News for 14 days.

E-Coliar should remain on the pet until stitches are removed to prevent licking or chewing.
